Overview
The New Hampshire Disability and Health Program (DHP) is excited to partner with community organizations to offer Living Well in the Community (LWIC)- a 10-week workshop that supports adults with disabilities in building healthier, more fulfilling lives. LWIC focuses on goal setting, problem-solving, and peer support, helping participants make meaningful changes that improve their overall well-being.
Funding for this project is provided in whole or in part through federally supported public health programming, and all resulting awards are subject to applicable federal, state, and USNH procurement requirements.
What You’ll Do
Selected New Hampshire based organizations will:
- Host one 10-week LWIC workshop for adults with disabilities
- Recruit 4–12 participants from your community
- Identify 1–2 facilitators to lead the workshop
- Support facilitators in completing LWIC facilitator training, which includes:
- Eight (8), 2-hour live Zoom sessions with an LWIC trainer
- Deliver the workshop following the LWIC model
Support & Technical Assistance
The NH Disability and Health Program will provide ongoing support to help you successfully implement LWIC. This includes:
- Guidance on recruitment strategies
- Support during facilitator training and workshop delivery
- Help with data collection and reporting requirements
- Availability to answer questions and problem-solve throughout the process
Data & Follow-Up
To help us understand the impact of the program, organizations will:
- Collect basic participant demographic information
- Ask participants to complete a short evaluation after the workshop
- Share this information with the NH Disability and Health Program
- All data must be handled in compliance with applicable data privacy, confidentiality, and security requirements
Funding
- $5,000 for hosting one LWIC workshop
- $3,000 for an additional (supplemental) workshop, if offered
- Payment is contingent upon successful completion of required deliverables, submission of required reports, and compliance with grant requirements
What to Include in Your Proposal
We’d love to learn more about your organization. To apply, organizations must serve New Hampshire residents. Please include:
- A brief overview of your organization and your work in the community
- Your ideas for recruiting participants
- Who your facilitators might be (or how you’ll identify them)
- Your experience (if any) with health or wellness programs
- Your commitment to accessibility, inclusion, and working with adults with disabilities
Timeline
- Proposals are due by June 30, 2026, at 11:59pm
- Proposals should be submitted to Emilia Poehlman, NH Disability and Health Program Manager
- Partnership notifications will be sent out by July 31, 2026
- Workshops must be implemented and evaluation data shared with the DHP team by May 1, 2027
Terms and Conditions
Governing Terms
Any award resulting from this RFP will be subject to:
- University System of New Hampshire Purchase Order Terms and Conditions (Version 1, updated 3.11.26) USNH Terms & Conditions
- Applicable federal grant regulations under 2 CFR Part 200
- All required federal flow down provisions, including but not limited to:
- Civil rights and non-discrimination requirements
- Debarment and suspension certifications
- Access to records
- Byrd Anti Lobbying Amendment
- ADA and Rehabilitation Act compliance
- Domestic preference requirements (when applicable)
In the event of a conflict, the order of precedence shall be governed by USNH policy. USNH Procurement forms and policies can be found on our website.
Federal Flow Down
Respondents must comply with, and flow down to any subcontractors, all applicable federal requirements, including those related to:
- Equal Employment Opportunity
- ADA and accessibility compliance
- Record retention and audits
- Prohibition on covered telecommunications equipment
- Non-discrimination across protected classes
Insurance Requirements
USNH insurance requirements are available on the USNH Procurement Services web page under Other Forms: USNH Insurance Requirements Within ten (10) business days after the Owner delivers a Notice of Acceptance, Agreement or Purchase Order, or prior to beginning work on campus, whichever is sooner, the successful Firm shall deliver to the Owner the Certificate of Insurance as specified in the USNH Insurance Requirements
Reservation of Rights
USNH and the New Hampshire Disability and Health Program reserve the right to:
- Reject any or all proposals
- Waive minor irregularities
- Request clarification or additional information
- Cancel this RFP at any time
