Policy System and Environmental Changes

NH DHP identifies Policy, System, and Environmental (PSE) Changes that will improve the health of NH adults with disabilities. Through the needs assessment, NH DHP identified gaps in healthcare and healthy living for adults with disabilities and is looking to partner with community organizations and pursue PSE changes in the following areas:

Food Pantry Accessibility Outreach

The Disability and Health Program (DHP) helps food pantries improve accessibility for people with disabilities. We assess their policies, systems, and spaces to identify strengths and areas for improvement.

Using tools like the Community Health Inclusion Index (CHII), we work with food pantries to develop strategies for better accessibility. This project seeks to help pantries make changes that improve access and create a welcoming environment for all. We also provide tools and guidance for food pantries to evaluate themselves and continue making improvements.
