NEGC Medical Home Webinar Series

The New England Genetics Collaborative (NEGC) is leading a Tri-State webinar series with the child health improvement partnerships in Maine (ME CHIP), New Hampshire (NHPIP) and Vermont (VCHIP) to highlight how a team approach to caring for children with genetic and complex conditions within the Patient Centered Medical Home (PCMH) will improve collaboration, coordination, and satisfaction for families and providers. The NEGC recently participated in the National AAP Project, Genetics in Primary Care Institute (GPCI).

Learning Objectives

  1. Understand why practices need to be concerned about strength based and family centered communication.
  2. Learn how practices can use better communication strategies to develop goals and improve the quality of life with the patients and families with special health care needs.
  3. Understand from a parent's perspective the important components of communication.

NEGC represents the six New England states as one of seven Regional Genetics Collaboratives supported by HRSA. NEGC's mission is to promote and improve the health and social well-being of those with inherited conditions through collaborations among public health professionals, private health professionals, educators, consumers and advocates.

Presenters

Dr. Barbara Frankowski, MD, MPH, FAAP
Dr. Frankowski has been a practicing pediatrician in Burlington, VT for almost 30 years, and is also very active in School Health, both in her community and at the national level, where she served as a member and Chair of the AAP Council on School Health for many years. Dr. Frankowski graduated from Johns Hopkins School of Medicine in Baltimore, MD; did her residency at Case Western Reserve University in Cleveland, OH; and received her MPH from Rochester University in Rochester, NY.

Dr. Jill Rinehart
Dr. Rinehart is a Pediatrician with Hagan, Rinehart Connolly Pediatrics in Burlington, VT. Her areas of expertise are Medical Home Initiatives, Family Centered Care and Care Coordination, and working with children with special health care needs. Dr. Rinehart is the Vice President of the Vermont chapter of American Academy of Pediatrics, the Medical Home Chapter Champion, and received the 2012 "Green Mountain Pediatrician of the Year" award for her contributions to child health in her state.

Tara Townsend
Tara Townsend is the Family Voices Coordinator at the RI Parent Information Network (RIPIN). Tara is also the mother of 3 children including one with an undiagnosed genetic condition. Tara is a former Special Education Teacher and has now been working for RIPIN for more than 5 years. Tara is a member of the New England Genetics Collaborative and sits on numerous work groups providing the parent perspective and advocates for families to create better outcomes.
