New Hampshire Inclusive Community Implementation Process

New Hampshire’s Disability and Health Program (DHP) is collaborating with the Granite YMCA to utilize National Center on Health, Physical Activity, and Disability's inclusive Community Implementation Process (NiCIP) to mobilize an Inclusive Health Coalition to implement, monitor, and evaluate an inclusion plan in Manchester and Goffstown, NH. Members of the IHC include a self-advocate with a disability, a healthcare provider, and staff from the Moore Center, AdaptX, Crotched Mountain Foundation, and Granite State Independent Living. This is a three year long project which will take place from August 2023-July 2026.
