NH DHHS Training Series for Case Managers and Service Coordinators
This DHHS training series is designed to give you the essential skills and knowledge you need to provide high-quality, person-centered case management and service coordination in Home and Community-Based Services (HCBS). Developed in partnership with the Human Services Research Institute, the University of Missouri, and the University of New Hampshire Institute on Disability, the series brings together national expertise for NH professionals.

- Foundations of Person-Centered Service Planning, Service Coordination, and Case Management
- New Hampshire Home and Community-Based Services (HCBS) Waivers
Collaborating to meet Person-Centered Thinking Training requirements
The Department of Health and Human Services, through the Bureau of Developmental Services (BDS) and the Bureau of Adult and Aging Services (BAAS), recognizes that the Foundations for Person Centered Service Planning, Service Coordination and Case Management course delivers comprehensive, foundational content in person-centered practices and meets the training requirements outlined in He-E 805.04(c)(6)(d) and He-M 506.05(c). This course offers a thorough orientation to the philosophy and practice of person-centeredness and reinforces New Hampshire’s language and expectations for how Case Managers and Service Coordinators apply person-centered thinking, planning, and practice in their daily work. Developed through a collaboration with HSRI which administers the National Center for Advancing Person Centered Practices and Systems (NCAPPS), the training aligns with and promotes nationally recognized standards.

All courses are hosted through the UNH Canvas Extend learning platform. You can complete them on your own schedule using a computer, tablet, or smartphone—anytime you have internet access.
