Frameworks for Implementing Person-Centered Planning

Navigating Choice & ChangeTraditional planning processes for adults with disabilities often focus on medical and financial needs, without addressing personal goals and preferences. Person-centered planning is a unique approach to planning for those who need services and supports that focuses on individualized choices and goals, and is based on a process of planning that promotes trust, cooperation, and creativity. Person-centered approaches have been shown to enhance community living options and increase quality of life.

This workshop offers an introduction to person-centered planning for adults. Participants will hear an overview of what we mean by a person-centered system of care, and learn about the Drexler/Sibbet Team Performance™ Model as a framework for the planning process. Presenters will share strategies for effective facilitation of person-centered planning meetings, as well as a variety of tools to assist in the process. Participants will explore some of these facilitative tools during hands-on group activities to gain an in-depth understanding of their purpose and utilization. Participants will leave this workshop with strategies and tools to begin implementing person-centered planning in their practices.

Who Should Attend

Case managers, social workers, counselors, educators, social service agency staff, nurses, long-term care specialists, clinicians, family members, and others involved in planning with older adults and adults with disabilities.

Participants in this workshop are eligible for 7 staff development credits/hours
Approved by the NH Chapter NASW for 6 Category I Continuing Education Credits for Social Workers.

Presenter(s)
Patty Cotton, MEd

Patty Cotton, M.Ed., offers educational programs, facilitation, and professional development services related to creative planning, strategic visioning, and individual and family-directed service designs. Patty has coordinated numerous federally-funded projects with a particular emphasis on model demonstration and system change in adult services. She has extensive background in process facilitation, service brokering, and development of natural support strategies. Patty has authored several publications on person-centered planning and natural supports and provides training at both the state and national levels.

Kate Crary

Kate Crary, B.S. joined the NH Institute for Health Policy and Practice in October of 2011. She currently serves as a Project Director with the Institute for Health Policy and Practice at UNH. Kate has worked on many programs with the Department of Health and Human Services in NH, and is a certified Long Term Care Ombudsman. In addition to this work, she is responsible for the design and implementation of numerous educational programs and trainings, focusing on an array of topics such as case management, person-centered planning, conflict management, care giving, transitional care, and other subjects.

Previously, Kate spent over a decade working in the landscape construction field as an Operations Manager for a local family-owned firm. Kate has experience in therapeutic art programming, children's theatre, caregiving, caregiver challenges, conflict resolution, and has an extensive background in project management, customer service, marketing, video production, hand-drawn videography and web design.
