Supporting Choice and Preferences: Tools for Caregivers

Two Women Sitting TogetherWhen providing care to another person, it can be challenging to know how to support personal preferences, care needs, and our own busy and often intense lives. During this workshop, we will explore tools you can use to collect, organize and prioritize important information about another individual. You will also gain skills to support important conversations with your care partner about their desires, so that care can be provided according to their wishes. We will discuss why this kind of information is important, how it can support better health outcomes, and how it can help us to balance the challenges of caregiving with other equally important responsibilities. The goal of this training is for you, the caregiver, to leave with tools you can use, and the skills to know when and how to use them to support your care partner.



Kate Crary

Program Manager, Institute for Health Policy & Practice

Kate Crary, B.S. joined the NH Institute for Health Policy and Practice in October of 2011. She currently serves as a Project Director with the Institute for Health Policy and Practice at UNH. Kate has worked on many programs with the Department of Health and Human Services in NH, and is a certified Long Term Care Ombudsman. In addition to this work, she is responsible for the design and implementation of numerous educational programs and trainings, focusing on an array of topics such as case management, person-centered planning, conflict management, care giving, transitional care, and other subjects.

Previously, Kate spent over a decade working in the landscape construction field as an Operations Manager for a local family-owned firm. Kate has experience in therapeutic art programming, children's theatre, caregiving, caregiver challenges, conflict resolution, and has an extensive background in project management, customer service, marketing, video production, hand-drawn videography and web design.
