Children's Behavioral Health Survey

May 2, 2018

The Institute on Disability is conducting a survey which will be used to help in the development of online courses in Evidence Based Practices in children’s behavioral health.  We are looking for participation from school staff (ie. educators, paraprofessionals, interventionists, etc.), mental health providers and any other professionals working with infants and children through age 18.  This survey consists of questions related to your current practices, previous trainings and your desire for additional training as well as areas of interest within children’s behavioral health for possible course development.

This survey is voluntary.  Should you choose to participate you can also choose to be entered into a drawing for a $50.00 Amazon gift certificate.  Participation in the drawing is not mandatory and the survey can be completed and submitted without participating in the drawing.

If you would like to participate, please read the consent form.  If you consent to the terms of the survey, you can click the link to begin.  If you have any additional questions about this survey or the Institute on Disabilities please feel free to contact Heidi Cloutier at 603-228-2084 ext. 41.
